What is your current spec and glyph set-up? Can you please provide some insight into why you chose the spec you did and how it enhances your game play and can benefit our raid? My Main Spec is Restoration <0/16/55> Glyphs are of: Water Mastery, Earth Shield, and Chain Heal. So far a Resto Shaman is my first and only healer, I chose the spec because in my opinion it provides a variety of healing supports such as Raid Heals or ESing and keeping up tanks. In addition to that I think it's fun. :P I've heard people say it's too easy, but I use all my healing spells in an appropriate balance instead of constant chain healing.
Professions

Mining <450/450> Soon to be Alchemy. Won't take much time at all.
Jewel Crafting <455/455>

Can you consistently make our raid times of 10pm-1am (Pacific Time)? Friday's and Saturdays are a no go but every other day I would be available, yes.
What is your previous raiding experience?

I've done OS 10/25, Ony 10/25, Naxx 10/25(Minus Thaddius on 25) ToC 10/25, Uld 10 (Minus Iron, Yogg, and Alganon) EoE 10, ICC 10 (5/12 progression) ICC 25 (4/12 progression) I've researched bosses passed my ICC progression but I've unfortunately haven't been with a group that has made it further.

Can you manage to: get out of void zones, avoid flame waves, maneuver around green clouds, and other such tools of destruction... On your own? Of course. :) It's not hard to move my character and still meet the demands of my class.
What will you bring to this guild? The only thing I think would benefit the guild from me is optional ideas I may have based off researching content if we're struggling with something. Other then that I'm very social....
What are you looking for in a guild? Progression, learning, and enjoyment! The new content has me very excited and I want to see more. Of course I also look for a guild to be realistic as far as life before game and that we all still need to have some fun as things get done.
What are you looking to avoid in a guild? Number one is guild drama. It's unnecessary. I'm also not interested in poor raid judgment. Ie; If we show NO progression in a raid after four hours please call the raid or change the strategy...
Do you have ventrilo installed, as well as a working mic? Absolutely.
Do you have a working computer that allows you to stay connected during raids? Yes.
Will you be bringing any alts in? If so, who? Not at this time, I have a couple other 80s but I don't feel they would be worth the guilds time until they're better geared. :P
How did you hear about STD, what prompted you to apply? ;) Mageroyal has been talking about the guilds potential with me. It sparked my interest and I can tell he enjoys the raids. Plus I fully trust his opinion on how I'd fit in with the guild as far as social or raid comfort. He says you're good people. :P
Give us some background on yourself and some of your non-WoW interests. I'm a Certified Nurse Aid waiting see if I've been accepted to go onto Licensed Practical Nursing and eventually Registered Nursing. Obviously due to the wait time I play WoW, but otherwise I spend my time with friends, animals and occasionally traveling to visit family. On top of that when it gets warmer I can't wait to get back to the beach!
